We offer self-pay services for those who do not have insurance or those who have an insurance carrier that is not one of our partners. The rates are as follows:

All payments are due before service. An invoice will be sent to the email on file for payments made the day of the appointment. If you have an appointment with us, check your email the day before and the day of your appointment for any balance due.

Cancellation: To avoid being charged for a miss appointment, you must send an email 72 hours prior to your appointment.

Rescheduling: To avoid being charged a miss appointment, we require you to reschedule 72 hours before your appointment to avoid charges of $75. Rescheduling will be done base on availability.

Medication refills: We require you to give 7-day notice for any refills. Anything after 7 days may require an appointment to be seen before processing a refill. All controlled substances need an appointment to be filled. We do not fill medication for lost scripts especially with controlled substances. You will be required to go to the emergency room to get evaluated if you lose your script for a controlled substance.

Labs: When labs are ordered for the purpose of treatment, it is essential to complete them prior to your next appointment to help your treatment process. Your treatment plan may get delayed if you do not follow the guidelines recommended. We use labs results to tailor your care. Genetic testing is not a mandated lab, but is recommended. However, other blood work that is part of your treatment is essential to complete to achieve your treatment goals.

Credit Card Payment: We require a payment method to process co-pay and charges that may relate to our services.

Patient Portal: You will be sent a link which you will use to create your patient portal account after completing the new patient appointment form. This portal facilitates communication, allows you to review lab results, and process payments.
